Hearing loss in your forties and fifties usually shows up as difficulty in noise long before quiet conversation becomes hard. We use video otoscopy to inspect the canal and eardrum, pure-tone audiometry to plot thresholds by air and bone conduction, tympanometry to assess middle-ear function, and speech audiometry to measure clarity. Together those four tests distinguish wax, glue ear, eustachian tube dysfunction and true sensorineural loss from one another — distinctions that matter, because the treatments are entirely different.
